Pain Puzzle – Adapting practice: \u00a0<\/strong>Infection r<\/strong>isk assessment and mitigation guide<\/strong><\/h1>\n

 <\/p>\n

This document provides a written record of the heightened infection control measures that this clinic has put into place to ensure the safety of all staff and patients during COVID-19.<\/strong><\/p>\n

Protection of staff and patients before they visit, and during home visits.<\/strong><\/p>\n

I have assessed my practice for risks outlined and put in additional processes as detailed below:<\/p>\n

Undertaken a risk assessment <\/strong>07\/01\/2022<\/p>\n

Heightened cleaning regimes<\/strong><\/p>\n

The portable couch will be cleaned before and after each treatment with sanitising wipes.<\/p>\n

Clinical equipment will also be cleaned before and after each treatment with sanitising wipes.<\/p>\n

Increased protection measures<\/strong><\/p>\n

No couch covers are used.<\/p>\n

Pillows and towels are provided by the patient.<\/p>\n

Payment is only accepted by card.<\/p>\n

PPE will be worn by the practitioner throughout face to face sessions \u2013 apron, gloves and mask.<\/p>\n

Patients are requested to wear a mask if possible.<\/p>\n

Put in place distancing measures<\/strong><\/p>\n

Distancing will be used where possible \u2013 particularly taking the case history. This may be taken by telephone if possible.<\/p>\n

Unless a chaperone is required observers of treatment are discouraged. Any additional persons in the treatment room will maintain a 2m distance.<\/p>\n

Staff Training<\/strong><\/p>\n

I have undergone training in hand cleansing and safe removal of PPE.<\/p>\n

I have also undergone COVID-19 safety training.<\/p>\n

Providing remote\/ telehealth consultations<\/strong><\/p>\n

I am providing first and follow-up appointments by telephone or video on request.<\/p>\n

 <\/p>\n

I have assessed the following areas of risk and put in place the following precautions<\/strong><\/p>\n

Pre-screening for risk before public\/patients appointment:<\/strong><\/p>\n

Telehealth \u2013 telephone or video case history and telephone support offered.<\/p>\n

Triage for COVID symptoms\/relatives with COVID \u2013 appointment confirmation email and telephone call\/text the day before.<\/p>\n

Screening:<\/p>\n

    \n
  • Covid-19 symptoms in patient or other household members \u2013 high temperature, persistent cough, change in sense of taste or smell<\/li>\n
  • Identifying clinically vulnerable patients or members of household.<\/li>\n
  • If been in contact with someone with suspected\/confirmed COVID-19 last 14 days.<\/li>\n
  • Identify if travelled to any countries outside the UK in last 14 days.<\/li>\n<\/ul>\n

    Information given:<\/p>\n

    Telehealth options \u2013 telephone or video consultation<\/p>\n

    Offer a chaperone.<\/p>\n

    Expectations of patient: Wear a mask, provide own towel and pillow, wash hands before session.<\/p>\n

    All documented in patient notes<\/p>\n

    Protecting members of staff<\/strong><\/p>\n

    PPE will be worn by the practitioner \u2013 gloves, apron, mask<\/p>\n

    A mask will be worn by the patient<\/p>\n

    Waste will be bagged and left at the home of the patient for\u00a0 disposal.<\/p>\n

    Wipes will be used to clean all clinical equipment.<\/p>\n

    If the patient or member of their household is in the clinically vulnerable category every effort will be made to arrange the appointment as the first visit of the clinic session.<\/p>\n

    Confirmed cases of COVID 19 amongst staff or patients<\/strong><\/p>\n

    If a patient reports symptoms of COVID-19 within 3 days of the appointment. The practitioner will be alerted to the onset of symptoms.<\/p>\n

    The practitioner will self-isolate and test as necessary in line with government guidelines.<\/p>\n

    Travelling to and from appointments<\/strong><\/p>\n

    All appointments will take place at patients homes.<\/p>\n

    I will travel in my own vehicle.<\/p>\n

    Entering and exiting homes<\/strong><\/p>\n

    Work clothing will be washed after each clinic session.<\/p>\n

    I will clean my hands with hand gel before entering and upon exiting their home.<\/p>\n

    Any unnecessary contact with surfaces will be kept to a minimum.<\/p>\n

    Reception and common areas<\/strong><\/p>\n

    There is no reception or common areas as this is a home visit service.<\/p>\n

    Contactless payment will be used wherever possible.<\/p>\n

    Social\/physical distancing measures in place<\/strong><\/p>\n

    Whenever possible the case history will be taken by telephone from the driveway of the patient and typed into the laptop.<\/p>\n

    Face to face will be limited to the examination and treatment as far as possible.<\/p>\n

    Face to face consultations (in-clinic room) <\/strong><\/p>\n

    No other family in the room unless they are there as chaperones. Any chaperones must wear a mask and remain socially distanced at 2m from the practitioner.<\/p>\n

     <\/p>\n

    Hygiene measures<\/strong><\/p>\n

    Increased sanitisation and cleaning<\/p>\n

    Alcohol gel will be used for hand cleansing before and after clinic sessions.<\/p>\n

    Cleanse hands \u2013 put on PPE \u2013 enter patient home \u2013 remove PPE \u2013 cleanse hands -leave patient home.<\/p>\n

    A wipeable bag will be used to carry equipment\/belongings one bag for each household \u2013 containing waste disposable bags, spare gloves if needed.<\/p>\n

    Another wipeable bag containing the payment processor and phone and clinical equipment, alcohol gel, wipes and tissues.<\/p>\n

    The portable couch will be wiped with alcohol wipes before and after each treatment.<\/p>\n

    All effort will be made to minimise contact of the practitioner with any unnecessary surfaces\/handles within the patient home.<\/p>\n

    Patients will be asked to wash or sanitise hands before and after treatment.<\/p>\n

    Aeration of rooms<\/strong><\/p>\n

    If possible and appropriate for confidentiality it is encouraged to keep the room ventilated with windows\/doors open.<\/p>\n

    Staff hand hygiene measures<\/strong><\/p>\n

    Staff will be bare below the elbow and use PPE and hand hygiene procedures as outlined above.<\/p>\n

    Respiratory and cough hygiene<\/strong><\/p>\n

    The practitioner will have tissues for single use and disposal for personal use as needed.<\/p>\n

    Cleaning rota\/regimes<\/strong><\/p>\n

    The couch and any equipment used will be cleaned after each patient visit.<\/p>\n

     <\/p>\n

    Personal Protective Equipment<\/strong><\/p>\n

    Clinicians will wear the following PPE<\/strong><\/p>\n

    The practitioner will wear a single-use plastic apron, fluid-resistant surgical mask and single-use gloves with each patient.<\/p>\n

    If the patient may be coughing or sneezing a visor will be available for use.<\/p>\n

    When will PPE be replaced<\/strong><\/p>\n

    The apron, gloves and mask will be disposed of at the home of each patient.<\/p>\n

    Patients will be asked to wear the following PPE<\/strong><\/p>\n

    Fluid-resistant surgical masks or cloth masks will be worn by patients unless they have a particular health condition that excludes them from wearing a face covering.<\/p>\n

    PPE disposal<\/strong><\/p>\n

    PPE, wipes and tissues will be bagged and left with the patient for disposal.<\/p>\n

     <\/p>\n

    Communicating with Patients<\/strong><\/p>\n

    Publishing your updated clinic policy<\/strong><\/p>\n

    The policy will be shared with patients on the database and with new patients when they register. It will also be made available on the website.<\/p>\n

    It will initially be shared in a newsletter and on the website and then with each appointment confirmation email.<\/p>\n

    \u00a0<\/em>It will be available on request as well.<\/p>\n

    Information on how you have adapted practice to mitigate risk<\/strong><\/p>\n

    Hygiene and consultation practices have been modified to take account of the latest government and profession-specific guidance. This will be updated in line with any new guidelines published.<\/p>\n

    Pre-appointment screening <\/strong><\/p>\n

    A form is sent to patients before a scheduled appointment.<\/p>\n

    Other patient communications<\/strong><\/p>\n

    All information will be communicated in a practice email to patients.<\/p>\n

    Patients will be asked to contact me if they develop symptoms within 3 days of the treatment.<\/p>\n
